Family Nurse Practitioner Baker joins Gouverneur Hospital’s Edwards team

EDWARDS – Family Nurse Practitioner (FNP) Brooke Baker has joined the medical team at Gouverneur Hospital’s Edwards Health Center, located at 8 Church St. 

Baker is Board Certified by the American Association of Nurse Practitioners. She earned her Advanced Family Nurse Practitioner degree through the University of Southern California, Los Angeles, CA; and her Advanced Practice Family Psychiatric Mental Health Nurse Practitioner certificate from the State University of New York at Buffalo. 

Baker began her career in healthcare as a Registered Nurse in the United States Navy, through which she is a two-time recipient of the Naval Medical Center Portsmouth Center Certificate of Excellence for Outstanding Patient Care. 

“Despite leaving the area for college and to serve in the Navy, I was welcomed back graciously when I first started practicing as an FNP in 2019. I have been fortunate enough to be supported by our community again and have been able to cultivate many positive and impactful relationships with patients, students, and universities, and community resources throughout my practice,” she said. 

“My goal is and always has been to help as many individuals as I can by delivering high-quality, holistic, evidence-based, patient-centered care, while implementing shared decision-making, and serving as a resource patients can utilize to achieve their own versions of health,” Baker continued. “I am passionate about providing preventative care across the lifespan and treating mental health, being proactive versus reactive, and emphasizing quality of life for patients and their families.” 

Being born and raised in the North Country has significantly shaped who Baker is in her personal and professional life.

“I have been surrounded by a community whose members have exemplified resilience, strength, kindness, compassion, empathy, and unity. I have been supported by our North Country residents in various ways throughout my life, and I am looking forward to continuing to give back to our area in my role as a nurse practitioner with St. Lawrence Health via Gouverneur Hospital’s Edwards Health Center,” she noted.

“I look forward to continuing to develop and expand the relationships I have within our region. I am excited to join the St. Lawrence Health medical team in its goal to provide excellent healthcare for the betterment of those throughout the North Country,” Baker said.  

As a Nurse Practitioner, Baker provides wellness exams, diagnoses illnesses, prescribes appropriate medications, reviews imaging results, interprets lab results, and educates her patients on treatment. She further develops, implements, and manages a plan of care for her patients with acute and chronic illnesses.
