Vaccination clinic for ages 12-18 planned Monday at Canton Central School

CANTON — Canton Central School will host a St. Lawrence County Public Health COVID-19 vaccination clinic for children ages 12-18 on Monday, May 24.

The clinic will be held in the high school auditorium from 8 to 11 a.m.

Vaccinations will be by appointment only. Register at https://apps2.health.ny.gov/doh2/applinks/cdmspr/2/counties?OpID=C2A1B9874D370266E0530A6C7C1591F8 .

The consent form, attached, must also be completed. Public Health prefers that these forms

be submitted to the school prior to the clinic.

Parents should have students submit the completed form to the school nurse's office by Friday.

Families may present the consent form on Monday at the clinic. However, children without a consent form cannot receive the vaccination.

Parents/Guardians are welcome to accompany their child during the clinic. Enter the

auditorium directly through the auditorium side door facing the flagpole.
