Children and youth with special health care needs program hosting transition event in Canton

CANTON – The Children and Youth with Special Health Care Needs program of St. Lawrence County is proud to announce an upcoming event that supports children with developmental delays or special health care needs as they transition to life after high school.

Transitioning from high school to adulthood can feel overwhelming, especially for individuals with developmental delays or special health care needs. Transition Triumph seeks to bridge this gap by providing a platform for attendees to learn from experts in various fields who specialize in supporting individuals with diverse needs.

Taking place on Tuesday, April 16, from 9 a.m. to 3 p.m. at the Woodcock Conference Suite in the Miller Campus Center of SUNY Canton, Transition Triumph will feature a lineup of respected speakers and invaluable resources.

Throughout the day, attendees will have the opportunity to engage with speakers who specialize in areas such as education, employment, independent living, medical, and community integration. These experts will offer practical advice, resources, and strategies to help individuals with developmental delays or special health care needs thrive as they embark on their post-high school journey.
