North Country This Week
CANTON – A new project at Hospice of St. Lawrence Valley will be presented to members of the Canton Rotary Club at its Oct. 24 meeting at the Best Western University Inn.
The endeavor called C.R.O.A.K. Book will be explained by Hospice volunteer Linda Potter and Mary Jones, director of family support services.
According to Potter, C.R.O.A.K. stands for Creating a Record and being Organized for Advance planning is Key.
“The workbook, which will be available for free in 2023, provides clear instructions and worksheets for people to understand the importance of completing advance planning and the necessary steps to be taken,” noted Potter.
Sections include Advance Health Care Planning, Social Media & Technology, Funeral & Burial Plans, Legacy, and Essential Documents.
“The book will be released in conjunction with the 40th anniversary of Hospice in St. Lawrence Valley and represents decades of experience of what forms and documents that if completed in advance can make life easier for loved ones,” said Potter.
To request a copy of the C.R.O.A.K. Book or to arrange for a presentation for your organization, contact Hospice of St. Lawrence Valley at 315-265-3105 or through their website, www.HospiceSLV.org.
