POTSDAM — North Country Matters host Donna talks with Stephanie Rogers, a Nurse Practitioner with Hospice of the St. Lawrence Valley about Palliative Care through the AIM (Advanced Illness Management) program that she calls the “best kept secret in St. Lawrence County.” 

Palliative care differs from Hospice care in that it helps patients and families manage the pain and distressing symptoms of one or more chronic diseases, coordinates care with medical providers, and educates about the likely progression of the disease(s). 

They discuss how to access palliative care, who would benefit from it, the costs, and how it supports the patient and the family with an advanced illness through home visits and advocacy.
