Hospice still offering all of its main services AIM, bereavement resources and outreach

Hospice of St. Lawrence Valley is still open and operating to serve patients and families during these extraordinary times.

Keeping with its mission, Hospice is still providing at-home Advance Illness Management service, grief/bereavment resources and services, and outreach and educational services.

The Advanced Illness Management program is essentially the old-fashioned house call serving patients who have been diagnosed with a chronic illness.

The physical needs of the patient are addressed through an in-home consultation provided by our nurse practitioner.

Depending on the stage of illness and other factors, the nurse practitioner will determine a schedule for follow-up visits.

This service is intended as an extra layer of support for the patient and family, our staff continuously collaborate with the patient’s primary care provider and specialists who are treating their illness.

The ability of AIM staff to coordinate medications and address pain and other symptoms in the home allows patients to remain in familiar surroundings and avoid traumatic trips to the emergency department.

AIM helps patients and families learn about their illness and effectively manage it to help improve quality of life.

Hospice is a leading resource for grief and bereavement in the community.

While support groups have been temporarily suspended, counseling services continue to be provided virtually and by phone.

In addition, Hospice has numerous resources on our website and in our library that are available free-of-charge to community members.

Grief has changed drastically and quickly.

Funeral practices have been limited to ensure the safety of surviving family members, however, it may significantly alter the grieving process, creating a lasting impact on those loved ones.

Hospice remains committed to addressing the changing needs of those affected by grief going forward.

Hospice will continue to offer outreach and education services electronically, including our popular Lunch & Learns.

Additional topics include grief cafe, coping tips to survive social distancing and various pod casts.

Although not in person, Hospice is working to create opportunities to offer these workshops virtually through Zoom, Facebook Live, YouTube and other online venues.
