OGDENSBURG -- Claxton-Hepburn Medical Center joined in National Case Management Week recently by highlighting its case management staff for services they provide for their patients.
“Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes,” according to the Case Management Society of America.
The recognition “is a way to be able to recognize our great case management team that works very hard every day, much of the time behind the scenes, to ensure our patients are safely led through their continuum of care,” said Mike Beldock, director of case management at Claxton-Hepburn.
