SUNY Canton offering care coordination certificate program in May

CANTON -- SUNY Canton, in partnership with the Fort Drum Regional Health Planning Organization and the North Country Initiative, is offering a care coordination certificate program beginning May 23.

The curriculum deals with "care transition," which refers to the movement patients make between healthcare practitioners and settings as their condition and needs change.

“The class is designed for both new students who are interested in the care management field, as well as existing coordination professionals from hospitals, behavioral health facilities or community-based organizations,” according to a news release from the school.

Classes will be from 5 to 8 p.m. every Tuesday from May 23 to Aug. 15 at SUNY Canton's Career Ready Education and Success Training Center (Halford Hall).

Tuition cost is $365, and students who are employed by a North Country Initiative partner are eligible for full tuition reimbursement upon successfully completing the course.

Space is limited.

Applications must be received by March 31.

Students must have at least a high school diploma in order to be eligible for selection.
