The Health Initiative will honor one health-conscious person with its Walter Lacks Award at their annual meeting in October, and nominations are being taken now.

The award, inaugurated last year, is to be given to a St. Lawrence County resident who has demonstrated enthusiasm and commitment to both personal wellness and to increasing wellness in others.

The award is named for an admirable community resident, Walter Lacks, who works tirelessly (well into his 80s) to improve his own health and the health of people here or across the nation and the globe.

The Health Initiative is accepting nominations from community members. Online nomination can be made at gethealthyslc.org. Downloadable forms to print, complete and mail in will also be available online at http://www.gethealthyslc.org/. Paper forms can be obtained by calling the office at 261-4760.

Nominations are due by Oct. 1. The award winner will be recognized at the Health Initiative's annual meeting on Oct. 18.
