A health insurance company is now offering plans for low- and moderate-income families and individuals in St. Lawrence County.
UnitedHealthcare Community Plan offers products for individuals and families who may qualify for Medicaid, Family Health Plus, Child Health Plus or New York Medicaid Advantage – or dual-eligible – plans.
These plans offer health coverage, including prevention, primary care, hospitalization, prescriptions and other services, often with little or no cost.
Members have access to various wellness programs, including nurse case management services for members who need additional assistance in managing chronic diseases and the “Healthy First Steps” program for expectant mothers and newborns.
The company also has outreach programs to remind and encourage people to seek preventive care.
The company has been offering similar coverages in other areas of the state.
