POTSDAM -- Canton-Potsdam Hospital will hold the next in its series of Partnering in Your Care monthly educational lunches on Friday, March 18, from noon to 1 p.m. in the second floor conference rooms A & B, 50 Leroy St.
Assistant Director of Quality/Case Management Ellen Nichols will talk about the transition of care from hospital to home, and the important role of the health care team in that process.
She will outline resources available in the community to assist with transitional and home care.
The lunch is free and open to the public, however seats are limited.
