POTSDAM -- Canton-Potsdam Hospital will hold the next in its series of Partnering in Your Care monthly educational lunches Tuesday, July 14, from noon to 1 p.m. in conference rooms A and B, 50 Leroy St.
Keri Allen, director of patient experience, will give a brief overview of the health care proxy form, as well as the new MOLST (medical order for life-sustaining treatment) form.
There will be an opportunity for questions and answers.
Both forms will be available.
Admission is free but registration is required.
