POTSDAM -- The Health Initiative, a collaboration by Massena Memorial Hospital, Canton-Potsdam Hospital and Claxton-Hepburn Medical Center to address county-wide health priorities, is offering a diabetes prevention program.

The evidence-based program, Prevent T2, lasts a year and focuses on instigating long-term changes for lasting results.

It is designed for people who have prediabetes or are at risk for type 2 diabetes, but who do not already have the disease.

According to the Health Initiative, a trained lifestyle coach leads the program to help you change aspects of your lifestyle, like eating healthier, reducing stress, and getting more physical activity.

The program also includes group support.

To enroll, or to find out more about the program log into http://gethealthyslc.org, or call Julie Cooke at the Health Initiative at 315-261-4760, ext. 228.
