OGDENSBURG -- The joint annual meeting of the Health Initiative and Bridge to Wellness organizations will be held Oct. 26, from 2:30-4:45 p.m. at Claxton Hepburn Medical Center’s Hess Auditorium, 214 King St.

There will be a presentation from Ian Grant from the Fort Drum Regional Health Planning Organization.

Walter Lacks Award, Patrick "Bird" Kerr Award, and Bridge to Wellness Community Health Champion Award will be presented.

The organization will also present the Health Initiative Annual Report and the Bridge to Wellness report.

Refreshments will be provided.

The Health Initiative was created out of a collaboration by Canton-Potsdam Hospital and Claxton-Hepburn Medical Center. Leaders from these institutions brought together over 200 individuals -- representing a broad spectrum of the community, including health, human service, business, government and educatino -- to conduct a thorough assessment of health in St. Lawrence County.

The steering committee identified three health priority areas that needed to be addressed county-wide: access to care, nutrition and physicial activity, and substance use, focusing on youth tobacco use.
