POTSDAM -- Canton-Potsdam Hospital’s monthly “Partnering in Your Care” educational luncheon, this time on chronic disease management, is set for Tuesday, Feb. 5 from noon to 1 p.m.
It will be in the second-floor conference rooms at the main campus, 50 Leroy St.
The program will be presented by Patricia Delosh, RN, Case Manager/Disease Management Specialist at CPH.
The hospital’s chronic disease management program was designed to assist patients with congestive obstructive pulmonary disorder, congestive heart failure, and diabetes to improve their quality of life.
“With the right coaching and personal commitment, individuals become empowered to take more control of their health at home, avoiding readmissions into a healthcare facility,” said Delosh.
“Taking medications as prescribed, eating a well-balanced diet, engaging in reasonable activities, and knowing the signs and symptoms to watch for are all influencers of success,” she said
At the presentation, Delosh will share information about resources and screenings available, and will answer questions.
